Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Alberta Health Card (AHCIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our AHCIP-covered services require a signed requisition from a certified Alberta healthcare provider, such as a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Alberta healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

My experiences with all the staff has been supportive and helpful. I appreciate the time and attention Dr Low gives to answer my diabetes questions given this is new to me. He is patient, kind and knowledgeable. I would also say the same for the dietician who helped me understand not only what to eat but also the math behind insulin injections. The admin staff are friendly and wonderful to chat with. It’s always an overall feel good environment, which is helpful when feeling overwhelmed with understanding my body’s needs.
