MyHealth Centre is now WELL Health Diagnostic Centre in Lindsay! We’re the same team with a new name, located in Ross Memorial Hospital on the 3rd floor (Yellow Wing Elevators). We provide OHIP-covered cardiology, nuclear medicine, and bone density services – with shorter wait times, faster report turn-around, and the highest standard of patient care. Request Appointment

Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

The emergency crew led by Mark were excellent in keeping me calm and transporting me to the hospital. Very kind and reassuring. The nurses were very quick to help with my pain and do an assessment of what I needed to have done. The x-ray technician was amazing and very gentle. Dr. Carruthers was incredibly kind and explained what was happening to me. The nurse in charge of me in Hall #6' was beyond belief in her understanding of me and also kind and gentle. I wish to thank everyone for all their assistance and care.
