We provide OHIP-covered cardiology and respirology services with shorter wait times, faster report turn-around, and the highest standard of patient care. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

I absolutely loved the staff the two separate days of my appts. From the front desk to getting my monitor attached, and my ultrasound. I had such an amazing experience. They made the nerves subside form the moment I walked in. Talked to each staff member with random questions to understand their job, to family, and I truly enjoyed it. Super friendly, very professional, and show nothing but respect to each individual that comes in. The office is immaculate, seating amount is perfect and well spaced. Thank you to the staff for making my first visit of the unknown comfortable.

I went to this location for my STRESS/ECHO test. I had specific requests due to my psychological conditions, Josh and Cahit were super accommodating. I have to say something about Josh though -- his discipline on following up with my appointments, explaining the procedure end to end, while ensuring I stay comfortable -- truly demonstrated outstanding customer experience for me. I wish you the best, Josh. To WELL HEALTH -- you guys are lucky to have someone like Josh working for your organization!! I went in on July 9 2026 to have a stress test and during the ultrasound and once I was on the treadmill i wasn’t feeling well. The cardiologist there that day showed me so much concern and sympathy she sent me to the ER right away! Because of her quick response I was admitted and had a stent put in my heart. THANK YOU SO MUCH!
