We provide OHIP-covered cardiology and respirology services with shorter wait times, faster report turn-around, and the highest standard of patient care. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

I’m not a local but Had my echo and my treadmill stress test here today for an aviation screening. I must say, from beginning to end everyone was very nice! The receptions was very pleasant. Ipsit did my echo and everyone involved including the student was very nice and welcoming. Cahit and Amanda did my treadmill stress test and they were both very professional. A great teacher for a great student, keep them both! Any questions they were allowed to answer they did professionally. The biggest thing here is they took their time and did things with care and had you in and out smoothly and on time. Something that is not found in many places anymore. Thank you again, I would make the trip back here to get testing done even if it’s not close to home.
