MyHealth Centre is now WELL Health Diagnostic Centre in Brampton! We’re the same team with a new name, located in the Chrysler Drive Centre at the corner of Williams Parkway and Chrysler Drive. We provide OHIP-covered nuclear cardiology with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment. Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail
or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except breast screenings for women ages 40-74. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

Thank you very much for allowing me to comment on my experience with this facility. I was more than pleased with my experience and with all the staff both clinical and administration. I was extremely nervous about having this procedure done, and everyone assured me that everything would be just fine and not to worry. Of course that is easier said than done. Everyone was so very polite from their first. Good morning, through the whole procedure for both days and my final exit after the tests were completed. I found everyone very professional and couldn’t have had a better experience.
