MyHealth Centre is now WELL Health Diagnostic Centre in Milton! We’re the same team with a new name, located in the Milton Professional Centre on Bronte Street South, just north of Derry Road. We provide OHIP-covered ultrasound, X-ray, mammography and bone mineral density services – with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment, except X-ray, which is provided on a walk-in basis. We look forward to seeing you!

Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

I had an excellent experience at Well Health clinic last Friday. I was there to get a Holter monitor put on by Ann, and she was incredibly professional, kind, and accommodating. She patiently answered all of my questions and made sure I felt comfortable with the device. This wasn't my first time meeting Anne—she previously did my ECG, and she has always been consistently careful, attentive, and wonderful with her patients. The positive experience starts right at the front desk. Michelle was very welcoming and efficient, ensuring that the wait time was minimal and everything ran smoothly. The entire clinic environment is incredibly inviting. I highly recommend them for their outstanding patient care!

The staff are absolutely fantastic, professional, and courteous. I was there for my 2nd trimester ultrasound and was treated with respect and kindness. I'm glad I was still allowed in even though I was late due to traffic. The Radiologists were very patient with the baby in my belly who already has quite a personality. She moved around so much but mostly tried to get away from the scanning device but she gave the Technician a thumbs up 👍! We, including my Husband, had a beautiful experience.
