Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

They were right on time and very thorough and very professional. Only complaint is that I had to enter the building by the back door while the handicap parking was at the front of the building. it was very difficult to walk that distance around the building for me as I am candicapped.

I had a PET scan booked for Feb. 19th at this location. This was my 5th one at this location in the past year. After much stress and worry over my small veins for the blood work, I am very happy to report everything went of without a glitch. Anupa was able to find my vein right away making this an easy process, and explained everything that was going on. Michael looked after my PET scan, and the same thing incurred.
