MyHealth Centre is now WELL Health Diagnostic Centre in downtown Toronto! We’re the same team with a new name, located in the 790 Bay Street Medical Professional Building on the southwest corner of Bay and College Street beside CIBC. We provide OHIP-covered ultrasound and X-ray services with shorter wait times, faster report turn-around, and the highest standard of patient care. Please call or chat live to request your appointment, except X-ray, which is provided on a walk-in basis. We look forward to seeing you!

Request Appointment

Step 1: Requisition

Download our requisition and obtain a signed copy from your referring healthcare provider*. If possible, E-mail or fax the signed requisition to our clinic.

Step 2: Schedule

All of our services require a scheduled appointment, except X-ray, which is provided on a walk-in basis**. Please schedule your appointment by telephone, or live chat located in the bottom-right corner of this website.

Step 3: Test Preparation

Please review our Test Preparation Guide prior to your appointment. Bring your Ontario Health Card (OHIP) and requisition to your appointment and arrive 15 minutes early to register.***

*Our OHIP-covered services require a signed requisition from a certified Ontario healthcare provider, except mammography breast imaging for women ages 40-74 at select locations. If you need a requisition, you can visit a family doctor, walk-in clinic or community health center.

*Already have a doctor’s requisition? We accept signed requisitions from any certified Ontario healthcare facility, including hospitals and independent health facilities.

**Our walk-in X-ray hours may differ from our hours of operation. Please scroll up to view our current walk-in X-ray hours.

***If you have mobility concerns, please ask your caregiver to accompany and assist you.

We will send your diagnostic report to your referring healthcare provider, who will follow up with you. We can send it to additional healthcare providers upon your request.

Booked a 11:10am appointment for an ultrasound, arrived at 5 to 11, the receptionist took the requisition and asked me to take a seat, then within 5 minutes, the nurse called my name. Got in, lie down, did the test, flipped and tested the other side. About 15 minutes later everything’s done. The nurse clearly mentioned that the result will be sent to my doctor and that was it. This place is clean, quick, efficient, on Bay Street right in the heart of downtown, next to the medical district where the big hospital are located. Recommend to use their online live chat to book your schedules due to the phone system was somehow had a glitch, that would cut you off and hung up automatically after they put you on hold. At the end it doesn’t effect their great service at all.
